This checklist is published by the Wisconsin Department of Public Instruction, but applies to eligibility in New Hampshire as well. Look at cognition, memory, reasoning, communication, problem solving, physical functions, psychosocial behavior (psychological or social functioning), executive functions (organizing, evaluating, and goal directed activities), speech and language, attention, abstract thinking, judgment, sensory/perceptual/motor activities, and information processing. Investigating these needs may require evaluations from specialists in several fields.
